We accept most major insurance plans. Below is our list of participating carriers, plus information on paying your bill and patient assistance programs.

Participating carriers

We participate with most major insurance plans at all of our locations, including:

  • Medicare
  • Excellus Blue Cross & Blue Shield — Commercial, Medicare Advantage, and Medicaid plans
  • United Healthcare — Commercial, Medicare Advantage, and Medicaid plans
  • Aetna — Commercial and Medicare Advantage plans
  • MVP Healthcare — Commercial and Medicare Advantage plans
  • Humana — Commercial and Medicare Advantage plans
  • Cigna — Commercial and Medicare Advantage plans
  • Wellcare
  • Fidelis
  • Medicaid

We do not participate with vision care plans. The care we provide is medical and surgical care for diseases of the retina, not routine eye care.

For plans we participate with, our office will bill your insurance directly. It is your responsibility to confirm coverage and any patient responsibility with your insurance company before your visit. If we do not participate with your plan, payment is expected in full at the time of service; our patient billing coordinators will provide a Good Faith Estimate in advance. If you don't see your plan above, please call our billing office — coverage and contracts change, and we may be able to confirm whether we now participate.

Urgent issues — please call

For urgent issues — sudden vision changes, flashes, floaters, a curtain in your vision, severe pain — please call us at (315) 445-8166 rather than messaging through the portal. Portal messages are reviewed during business hours and are not appropriate for time-sensitive concerns.

Medicare and patient responsibility

As participating providers in the Medicare program, we are required by federal law to collect unmet deductible and coinsurance amounts at the time of service. If you have questions about your patient responsibility, our billing team can review your specific coverage with you — call (315) 445-8179.

Patient assistance — Good Days Chronic Disease Fund

The Good Days fund is a national non-profit that has historically helped Medicare and Medicare Advantage patients with the cost of injection coinsurance, based on financial need. Funding levels change. Learn more about the program's current status →

CareCredit is a third-party healthcare credit card that can be used to pay for out-of-pocket medical costs over time. We accept CareCredit at all of our offices.
